Service Description
PLEASE READ: This is for first-time clients only. Returning clients see the link below. You will be consulting with our nurse practitioner, Maryjane Awatin. After consultation, scheduling a Pre-Service Evaluation (GOOD FAITH EXAM) is required to obtain Botox treatment. During your initial meeting with Maryjane - NP, she will evaluate all relevant details and medical requirements to ensure a safe and effective procedure. Following the exam, we will establish the treatment date, and in certain instances, the treatment may be administered on the same day. We recommend considering this when selecting your appointment time. Please call if the preferred date and time is not available. Cancellation Policy
BOOKING & CANCELLATION POLICY A $50 deposit or full payment is required to secure your appointment. Online payment is accepted at the time of booking. Appointments may be canceled or rescheduled up to 24 hours before the scheduled start time. Cancellations or rescheduling requests made less than 24 hours in advance, as well as no-shows, will result in the forfeiture of the $50 deposit. If sufficient notice (24 hours or more) is provided, the deposit may be refunded or applied to a rescheduled appointment. By completing your booking, you acknowledge and agree to this cancellation and payment policy.
