PLEASE READ: This is for first-time clients only. Returning clients see the link below. You will be consulting with our nurse practitioner, Maryjane Awatin. After consultation, scheduling a Pre-Service Evaluation (GOOD FAITH EXAM) is required to obtain Botox treatment. During your initial meeting with Maryjane - NP, she will evaluate all relevant details and medical requirements to ensure a safe and effective procedure. Following the exam, we will establish the treatment date, and in certain instances, the treatment may be administered on the same day. We recommend considering this when selecting your appointment time. Please call if the preferred date and time is not available.
